Integrated Services for Children with Special Health Care
Needs
Consistent with the President’s New Freedom Initiative, families of children
with special health care needs and youth with special health care needs will
partner in decision making at all levels and be satisfied with the services they
receive. Activities for achieving this goal include completion of the Road Map
to Success document, creation of model transition information packets for youth
and families and revisions to the searchable database, the Service Tapestry.
