~ A completed project ~ MATP is a four year project to develop a statewide collaborative program that will enhance the transition of adolescents with special health care needs from high school to work and/or higher education. The program is designed to be accessible to all types of human diversity. The project includes extensive adolescent input in identifying service needs in all aspects of social, financial, educational, work and physical life. Funding Sources:
