The Maine Health Care Notebook

The Maine Health Care Notebook was designed by members of the Family Advisory Council to Maine's Children with Special Health Needs program.

In caring for your child with special health needs, you may get information and paperwork from many sources. You may also want a place to keep track of medications, treatments, etc. that are part of the medical care your child receives. The MaineCare Notebook coverMaintaining a health care notebook will help you keep the information organized in a central location. Developing a health care notebook for your child will also make it easier for you to share information with others who are part of your child's care team.

To download The Maine Health Care Notebook (MS Word document) to your desktop right-mouse-click (control+mouse-click on a Macintosh) on the text above or on the icon to the right. Open it using Microsoft Word.

Use this link to print the Maine Health Care Notebook cover (.pdf document).

You can then personalize your notebook by printing off the pages you feel will best meet you and your child's needs. The pages can be stored in a three-ring binder for easy access.
